Do oral probiotics actually work or are they a fad?

Learn what research says about oral probiotics for cavities, gum health and bad breath, plus why they should not replace proven daily oral care.

Oral probiotics are not just a fad, but the evidence is still limited. Some studies suggest certain probiotic strains may lower levels of bacteria linked to cavities and may help with bad breath or gum health. Results vary by strain and product, and probiotics have not been proven to prevent cavities or treat gum disease. They should be used as an add-on to daily oral care, not a replacement for brushing, cleaning between your teeth or regular dental visits.

What are oral probiotics? 

Probiotics contain live microorganisms intended to support health. You may know them from yogurt or gut-health supplements. Oral probiotic products use strains intended for the mouth and may come as lozenges or chewables. The goal is to influence the mix of microbes in your mouth, but the effects depend on the strain and product. 


How do they work? 

Reviews in the Journal of Pharmacy and Bioallied Sciences and European Journal of Dentistry describe several ways oral probiotics may work. They may interact with other microbes, influence immune responses and compete with harmful bacteria for places to attach in the mouth. 


There are limits. Some probiotic strains have been found in the mouth while people were using them, but how well they stay there varies by strain, product and person. Long-term colonization is still uncertain.¹˒² 


What does the research say about cavities? 

One of the main bacteria behind tooth decay is Streptococcus mutans, or S. mutans for short. Researchers have been asking a simple question: Can probiotics help keep it in check? A review in the Journal of Pharmacy and Bioallied Sciences found the answer depends a lot on which strain you're talking about; certain strains, including forms of L. reuteri and L. rhamnosus GG, showed promising results.¹ 


A 2023 systematic review in Pharmaceuticals adds to that picture, finding short-term drops in salivary S. mutans in small studies of children.³ That's encouraging, but lowering the amount of one bacterium doesn't prove that probiotics prevent cavities. 


The 2022 review also pointed to a study where a probiotic tablet helped saliva buffer acid better.¹ It's an interesting thread for researchers to pull on, but on its own, it doesn't prove the probiotic prevents tooth decay. 


Can they help with gum disease? 

Some studies have tested probiotics alongside standard gum-disease treatment. A 2023 systematic review found that certain probiotic strains were linked to improvements in measures such as gum bleeding or pocket depth in some studies.³ 


That does not mean probiotics treat or cure gum disease. The studies used different strains and treatment plans, and more research is needed to know how useful probiotics are for gum health. 


What about bad breath? 

There is some promising research. A 2023 systematic review included an eight-week trial of tablets containing Weissella cibaria. The trial reported improvements in subjective bad breath and oral-health-related quality of life.³ 


That is encouraging, but it does not mean every probiotic strain or product will work for bad breath. 


What are the limits of oral probiotics? 

Oral probiotics are promising, but the science is still developing. Different studies use different strains, doses, products and groups of people. Many studies are also small or short.  


A 2023 systematic review concluded that better evidence is still needed on what happens with longer use and how probiotics behave in the mouth over time.³ The National Institute of Dental and Craniofacial Research advises using fluoride toothpaste two times each day, cleaning the spaces between your teeth and getting regular dental exams and cleanings.⁴

Are oral probiotics safe? 

The strains used in oral probiotics are generally considered well-tolerated. Your dental provider can help you figure out if a product is right for you, especially if you take medication or have a health condition. 


Do oral probiotics work better for some people than others?  

Research suggests results can vary based on the existing bacteria in your mouth. People with higher levels of harmful bacteria may see more noticeable changes. Your dental provider can assess your baseline oral health and help you set realistic expectations. 


How long before I might notice a difference? 

There is no set timeline. Studies have used different products and treatment periods, so the results cannot be used to predict exactly when you might notice a change. 


Do oral probiotics interact with toothpaste or mouthwash?  

Antibacterial mouthwashes may reduce the effectiveness of oral probiotics by killing the probiotic bacteria before they can settle in your mouth. If you use both, timing may matter. Ask your dental provider whether the products you use are compatible.
