How does fluoride toothpaste work?
Fluoride toothpaste works by rebuilding and protecting your enamel. The National Institute of Dental and Craniofacial Research (NIDCR) says fluoride strengthens your enamel and helps reverse early decay by putting lost minerals back¹. The Centers for Disease Control and Prevention (CDC) says fluoride replaces those minerals after acid wears them away.²
Here's what makes fluoride unique. When fluoride meets your enamel, it creates a new crystal that's harder than what was there before. The American Dental Association (ADA) explains that fluoride converts the natural mineral in your enamel into a more acid-resistant form.³ That makes your teeth tougher against future acid attacks.
Is fluoride toothpaste safe?
The CDC says fluoride dental products are regulated as drugs by the FDA. They are safe and effective when used as directed.² The ADA says all toothpastes with a cavity protection claim and the ADA Seal must contain fluoride.⁴
One thing to watch: too much fluoride during tooth development can cause dental fluorosis.¹ Fluorosis shows up as white spots or streaks on teeth. The NIDCR advises not using fluoride toothpaste for children under age 2 unless a dental provider or doctor says otherwise.¹
Who benefits most from fluoride toothpaste?
Some people get the most out of fluoride. These include people with a high cavity risk, people with dry mouth and those who don't have access to fluoridated water. Your dental provider can check your cavity risk and help you decide.
How does hydroxyapatite toothpaste work?
Hydroxyapatite (say it: hi-drox-ee-AP-uh-tite) is a mineral. It makes up the bulk of your tooth enamel and dentin naturally. Toothpastes with hydroxyapatite use a lab-made version of this same mineral to fill in small gaps and smooth the surface of your teeth.
Because it's built from the same material as your enamel, the idea is that it slots right in. This is sometimes called a biomimetic approach. That just means it copies what your body already does.
What does the research say?
Hydroxyapatite has a growing body of research behind it. Some studies show it can remineralize enamel and reduce sensitivity.⁵ But it doesn't have the same scale of long-term, population-wide data that fluoride has built up over decades. That gap doesn't mean it doesn't work. It means the evidence is still developing.
Who might prefer hydroxyapatite toothpaste?
Hydroxyapatite is worth considering in a few situations. Parents of young children may prefer it since fluoride ingestion is a concern at that age.³ People who want a fluoride-free routine for personal reasons may also find it a good fit. It may also suit those with mild sensitivity, since it can help smooth and cover exposed areas.
Can you use both?
Some people use one in the morning and the other at night. Others alternate by week. There's no clinical rule against using both. If you're curious about this approach, ask your dental provider what makes sense for your teeth.
Which one should you choose?
There's no single right answer. Fluoride has the longer, larger evidence base.¹,³ It's the most widely used fluoride product in the U.S., says the CDC.² Hydroxyapatite is a solid fluoride-free option with growing support, especially for lower-risk patients or young children.
Think of it this way: fluoride and hydroxyapatite are different tools. One isn't better. Each fits a different set of needs.

Can hydroxyapatite toothpaste help with tooth sensitivity?
Hydroxyapatite is the same mineral that makes up your enamel and dentin. Because of that, it can fill in tiny surface gaps that expose the nerve endings responsible for sensitivity. Some people notice less reactivity to cold, heat or sweets with regular use. If sensitivity is your main concern, your dental provider can help you decide whether a hydroxyapatite toothpaste, a fluoride sensitivity formula or another approach is the better fit.
Is hydroxyapatite toothpaste safe to swallow?
Because hydroxyapatite is already a mineral found in your body, it's generally considered low risk if a small amount is swallowed. That makes it appealing for toddlers. But your dental provider is the right person to advise you on what's safe for your child's age and stage.
Do over-the-counter hydroxyapatite toothpastes have an ADA Seal?
The ADA Seal for cavity protection requires fluoride.4 Hydroxyapatite toothpastes won't carry that specific seal. That doesn't mean they're unsafe. It means the ADA's current cavity-protection standard is built around fluoride. Ask your Aspen Dental provider which products they recommend.
