Patient eligibility
We provide dental care to eligible Illinois residents who need it most. You may qualify if you meet the following criteria:
Residency: You must live in Illinois.
Medicaid: You must be eligible or enrolled in Medicaid.
Income: Patients with annual household income at or below these limits could qualify for free care:
1 person: $31,920
2 people: $43,280
3 people: $54,640
4 people: $66,000
Dental need: Missing teeth, cracked teeth, need a tooth replaced or wear dentures.
